I, ____________________________________ (participant’s name), hereby voluntarily acknowledge and agree that participation in sports, recreational activities, events, and other associated activities at BLUE FURY SPORTS COMPLEX entails inherent risks and hazards, including but not limited to personal injury, physical harm, emotional and financial damage, illness, food-related illnesses, allergic reactions, loss of personal belongings, and even death. I fully comprehend and voluntarily accept all such risks.

GENERAL RELEASE AND WAIVER:
I expressly release, discharge, and hold harmless BLUE FURY SPORTS COMPLEX, its owners, affiliates, directors, employees, agents, representatives, partners, volunteers, sponsors, contractors, and any other entity or individual associated with the Complex from any and all liability, claims, damages, lawsuits, and expenses (including legal fees) resulting from my participation or attendance at the Complex.

SPECIFIC RISKS ACKNOWLEDGMENT:
I specifically acknowledge and accept risks involving but not limited to:

• Injuries resulting from collisions with other participants or objects.

• Injuries due to falls, improper use or malfunction of equipment.

• Illness or injuries related to consumption of food or beverages provided or sold at the Complex, including food poisoning and allergic reactions.

• Risks associated with any recreational activity offered or sponsored by the Complex.

• Loss, theft, or damage of personal belongings or property.

PERSONAL CONDUCT AND RESPONSIBILITY:
I agree to act responsibly and comply fully with all posted rules, policies, guidelines, and instructions provided by BLUE FURY SPORTS COMPLEX. I accept full responsibility for my actions and any consequences thereof, including ensuring I am physically and medically fit to participate in activities offered.

MEDICAL AUTHORIZATION:
In the event of a medical emergency, I authorize BLUE FURY SPORTS COMPLEX, its staff, and agents to administer first aid, seek professional medical assistance, or arrange for emergency transportation. I acknowledge that I am fully responsible for all medical expenses incurred as a result.

MEDIA RELEASE:
I consent to the use, reproduction, editing, broadcasting, publication, and distribution by BLUE FURY SPORTS COMPLEX of my image, voice, and/or likeness in photographs, video, or other media formats for promotional, educational, or advertising purposes without further compensation.

MINORS:
If the participant is a minor (under 18 years), I, as the parent or legal guardian, fully agree and accept all conditions herein on behalf of the minor and affirm that the minor is fit to participate in all activities.

FOOD & BEVERAGE DISCLAIMER:
BLUE FURY SPORTS COMPLEX is not responsible or liable for any illness, allergic reactions, or other health issues resulting from food or beverages consumed on its premises, including items sold by third-party vendors or food trucks. Participants are responsible for making informed decisions regarding food consumption.

SURVEILLANCE NOTICE:
Participants acknowledge that BLUE FURY SPORTS COMPLEX uses security cameras and other recording devices throughout its premises for safety and security purposes. By signing, participants consent to being recorded and waive any privacy claims related to such recordings.

PERSONAL PROPERTY DISCLAIMER:
BLUE FURY SPORTS COMPLEX is not responsible for any lost, stolen, or damaged personal items or property. Participants should secure their belongings at all times and assume full responsibility for their personal property.

GOVERNING LAW:
This waiver shall be governed and interpreted under the laws of the State of Florida. Any dispute arising from participation at BLUE FURY SPORTS COMPLEX shall be resolved exclusively in the courts of Miami-Dade County, Florida.

AGREEMENT AND ACCEPTANCE:
I have read, understand, and voluntarily sign this waiver and release agreement, agreeing to its terms fully and without reservation.
